Chinese fishing boat detained in Japan: Xinhua

BEIJING (Reuters) - Japan's coastguard detained a Chinese fishing boat within Japanese waters on Sunday evening, China's Xinhua news agency said, citing the Chinese consulate general in Fukuoka, a city in southwestern Japan.

The captain of the boat, registered in the southeastern Chinese province of Fujian, and two crew members have been brought to the southern Japanese city of Kagoshima for questioning, Xinhua said.

It said the captain had admitted he was in Japanese waters.

The incident comes just months after anti-Japanese protests erupted in many Chinese cities over disputed islands near Taiwan, known as the Diaoyu in Chinese and Senkaku in Japanese. Those tensions have cooled but could flare up if stoked by nationalists on either side.

A spokesperson for the Japanese embassy in Beijing could not be reached for comment late on Sunday evening.

The Chinese fishing fleet tends to range far into the waters east of China to offset depleted stocks closer to shore.

Editorial Media 'release windows' are increasingly archaic, futile and hostile

Last week's Downton Abbey broadcast debacle, while not representative of the world's most acute problems, unnervingly illustrated one of the many ways that media companies fail to understand markets, technology and day-to-day consumer realities. The hit show ended its third season on December 25th with an extreme plot development. The season was broadcast only to its British audience, while American viewers were waiting until 2013 to clap their eyes on the latest round of shows. Problem was, of course, that the entire non-UK audience had the whole third season spoiled by instant social buzz and UK-generated web reviews of the final dramatic denouement.

If technology does nothing else, it destroys boundaries of all sorts -- between countries, time zones, populations, affiliations and cultural circumstances. Media companies that distribute their products as if those borders still held sway seem increasingly clueless and hostile to their ever more empowered audiences.

Dallas and Washington Meet in the Karma Bowl

So this is it, the regular season finale between the Dallas Cowboys and the Washington Redskins.? The scene is set in snowy Washington,D.C.on prime time. ?These two teams meet to play what I?ll call the ?Karma Bowl?.

Picture this, before the season starts, the NFL and ironically, New York Giants owner John Mara, who is the chairman of NFL?s management committee, was among those who led the charge to take away salary-cap space from the Cowboys and Redskins for an uncapped season. ?After this hit was taken by both teams, ?experts? everywhere?speculated?that this would be the beginning of the end to these two franchises for the 2012 season as it handicapped them during the free agency period. ?After losing appeals, both the Cowboys and Redskins were counted out for any playoff appearances.

Fast forward to tonight?s game, these two teams who were victims of shady NFL dealings are now meeting for a win and get in scenario, for the most post. ?It is a must win and get in for the Cowboys. ?Two teams that were predicted to not go far, had the most to overcome from the start are now the teams who meet on Sunday Night Football for the NFC East Division Title. ?Wonder how bad that burns John Mara and the New York Giants? ?Especially since all off-season and even early on in the season Cowboy fans heard how the Giants were going to repeat.

The Karma Bowl will not be a disappointment to both fan bases. ?It has all the drama the NFL loves, along with a heated rivalry and a win or go home scenario. ?This is what the NFL is about. ?This is why the NFL changed the schedule to have division rivals go head to head in week 17. ?This game will be a slug fest no matter how bad either team is hurting with injuries, everyone will show up to prove the experts and the NFL themselves wrong in counting either team out early. ?This is December football and this is what karma is all about!

CCOTY 1968 Nomination: NSU Ro80?The Tragic Automotive ...

In the Pantheon of automotive gods and godesses, the Ro80 occupies a long-guaranteed spot. Simply put, it is one of the boldest, most influential and revolutionary cars ever?despite being a dismal failure. Its tragic story comes straight from the Greek legends; perhaps it should have been called the Ikarus.

NSU was a small maker of little cars and motorcycles. Both their 600cc Prinz and slightly larger 1000/1200 were notable for their Corvair-like looks, as well as for the air-cooled rear engine that was tucked in close to the rear wheels. But as incomes grew throughout the ?60s, the German (and European) market was quickly expanding into larger classes, and NSU made a bold double bet.

Their decision to expand up-market with a new four-door sedan was certainly ambitious. And got even more so: What was initially intended as a Ford Taunus competitor was later scaled up to directly compete against the smaller Mercedes models and BMW?s Neue Klasse. Ambitious indeed?but that was the relatively easy part.

Having signed a development contract with Felix Wankel, NSU co-developed what became the modern rotary engine (Wankel?s original design, which included a rotating engine case, was scrapped). Their new engine debuted in the limited-production NSU Spider (seen above in an interesting contemporary street shot).

Only 2,375 of the single-rotor, 50 hp cars were built, more or less as rolling test labs. It wasn?t long before the engine exhibited problems, mostly with the tricky apex seals?something that should have given NSU pause about powering the Ro80 with a larger, two-rotor version placed ahead of the front (driving) wheels.

The Ro80?s 995cc, twin-rotor engine was rated at 113 hp (115 PS). Because of the rotary?s tendency to run rough on overrun with a manual transmission, NSU turned to a Saxomat semi-automatic transmission, built by Fichtel and Sachs (and also offered by Porsche as the Sportomatic). It combined an shift-lever actuated automatic clutch with a torque convertor. Although there was no clutch pedal, one shifted through the three gears manually, much like the automatic stick shift found in some contemporary VW Beetles.

Inboard front disc brakes rounded out the very ambitious drive train, which was designed to shine on the German autobahns that were mostly uncrowded in the early- and mid-sixties. Unfortunately, the drive train was less ideally suited to dense traffic and city driving.

You already know where this is going: The Ro80?s engine was woefully underdeveloped in terms of durability. It became a nightmare of warranty costs and earned such a bad reputation that NSU was eventually forced to accept a takeover by Audi. That said, let?s focus instead on one of its indisputably superb qualities: The very enduring influence of its superb body design.

The Ro80 body design was an in-house job executed by Claus Luthe. It is utterly brilliant, even if some of its proportions seem a wee bit eccentric from certain angles. But that just adds to its allure, at least in the eyes of its admirers.

Needless to say, you?re looking at the mother of all Audis here?and the Taurus and, to some extent, just about every modern sedan. The drooping front nose, the overall wedge shape ending in a tall tail, the gracefully arching roof. The R080?s vast and enduring influence is unmistakable. And its aerodynamic Cd of 0.355 was excellent for the times.

The Ro80 was the significant link between that other goddess, the 1955 Citroen DS, and just about everything that followed. The ?utterly uncompromising shape of the DS was hardly conducive to cribbing; it was certainly revered, but mostly at a distance. In contrast, the R080?s design was seen as a more pragmatic way forward, as it incorporated the key advantages of the Citroen?s long-wheelbase and aerodynamic design in a more palatable and practical solution.

Only 37,398 Ro80s were built over its decade-long lifespan. NSU tried valiantly to address the severe issues involving engine life (some engines showed apex-seal wear after as little as 15,000 miles and were worn out by 30,000).?Still, it was a losing battle. Although it would later be fought with relatively greater success by Mazda, similar issues plagued their rotaries as well.

Many frustrated?owners refitted their Ro80s with the little Ford V4,?which was the only engine that fit. It was an ironic choice: The rotary?s biggest asset was turbine-smooth running, and the Ford V4 was one of the roughest engines ever built. Of course, Saab took the same road with their two-stroke 96, but did so officially.

A modest number of Ro80s were sold in the U.S. I?ve never seen one, nor do I expect to, so the prospect of an in-depth?write up?here is not terribly likely. But ateupwithmotor just recently published his article on the Ro80, and it is (as always) very comprehensive?and it?s right on time for the Ro80?s CCOTY nomination, as its historical value is more appreciated than ever (during the late seventies, one could hardly give an Ro80 away). The Ro80 was COTY in Europe in 1968, beating out the Fiat 125 (second) and Jensen FF (AWD).

The New Year is, for many, a time of change and resolutions. Be thinner! Be richer! Meet the love of your life!

For others, our hopes and dreams are more measurable. Whether it?s a 10 percent pay raise you?re after or moving one step closer to the corner office, forget job-hopping.?In 2013, staying put and amping up your performance at work is the way to make it happen.

?The prevailing wisdom has been that to get ahead, you should learn something from one company and move on?and up?at the next,? says Brian Kropp, a managing director at?CEB, an executive advisory firm?which offers data analysis of more than 50,000 employee surveys from 10,000 organizations. ?But that only produces short-term effects. In the new workplace we?re seeing greater emphasis on relationships,? he says, which means veteran employees are at a far greater advantage. According to CEB research,?longer-tenured workers are beginning to rise to positions of success more quickly than those who move every few years.

So what does this mean for 2013 career resolutions? Ditch the job boards and set to work making yourself an indispensable employee.

?Being indispensable is about being the best,? says Lucy Leske, vice president and co-director,?education?practice at the?executive search firm, Witt/Kieffer. ?If you?re always striving to be a better, more valuable contributor, people will inevitably take note and you?will?get ahead.?

Without further pontification, seven simple strategies to becoming indispensable in 2013.

Be flexible
?The odds are that the way you?ll do work on January 1st?won?t be the way you?ll be doing work on December 31st,? says Kropp. According to?CEB?research, more than 50 percent?of employees say they have experienced ?significant change? at work in the past 12 months, from reorganizations to new workflows to massive layoffs. ?Make sure that your boss sees you are someone who can get the job done no matter what?s happening around you.?

Stay current
?If you?re not regularly reading about industry trends in trade, business and general publications, checking out online sources and staying current on trends in your industry, you?re compromising your career growth,? says Leske. ?Keeping up on trends, but more importantly, being able to apply those trends to your organization, demonstrates your understanding if its place within the industry.?

Don?t be a loner
In the new workplace, 40 percent of employees work with more than 20 people on a given day, and more than 80 percent work with 10 according to?CEB?research. ?The idea that you can be an individual contributor and be successful is an idea of the past,? says Kropp. ?Fitting within the network of the workplace is a part of the new definition of a great employee.?

Be a thought leader
All of that knowledge you?ve gained by reading up on the industry? Leske says to make a habit of sharing it. ?Write articles, make presentations, serve on panels or blog,? she says. ?People need to have confidence in you that you know what you?re doing and that you?re willing to use it to help other peoples? problems.?

Prioritize
?It?s really easy to add more things to your to-do list but just as critical?if not more so?to know what to take off,? says Kropp. It?s no secret that work can be an overwhelming place, particularly in a post-recession environment where Kropp says the number of direct reports answering to any given manager has increased by an average of 50 percent in the past five years. Good decision making, delegating and prioritization are the signs of an effective leader, no matter your position within the organizational matrix.

Seek opportunities for management experience
Speaking of managers, Leske advises that you actively pursue any opportunity for managing employees, no matter how small and trivial (or large and daunting) the task may seem. ?There?s a difference between begging for these opportunities and raising your hand,? she warns, ?but if someone says there?s a job to be done, raise your hand first and ask for help later. The biggest mistake is passing up the opportunity.?

Make friends with the IT guy
The average number of work-related emails we receive each day has increased fourfold since 2005, underscoring the explosive importance of technology in the office. This makes the IT department not just a vital team in the workforce, but an essential ally to any employee reaching for success as with their help you can avoid unnecessary downtime due to tech failures.

But Kropp adds that it isn?t just the IT team who have become increasingly important within the workplace. ?Making friends with admins is an important move as well,? he says. Speedy boson machine could bridge classic and quantum computing

A new type of machine could rival quantum computers in exceeding the power of classical computers, researchers say.

Quantum computers?rely on the bizarre properties of atoms and the other construction blocks of the universe. The world is a fuzzy place at its very smallest levels ? in this realm where?quantum physics?dominates, things can seemingly exist in two places at once or spin in opposite directions at the same time.

The new computers rely on "boson" particles, and resemble quantum computers, which differ from traditional computers in important ways. Normal computers represent data as ones and zeroes, binary digits known as bits that are expressed by flicking switch-like transistors on or off. Quantum computers, however, use quantum bits, or qubits (pronouced "cue-bits"), that can be on and off at the same time, a state known as "superposition."

This allows the machines to carry out two calculations simultaneously. Quantum physics permits such behavior because it allows for particles that can exist in two places at once or spin in opposite directions at the same time.?[Experiment Demonstrates Possibility of Quantum Internet]

In principle, quantum computers could solve certain problems much faster than can?classical computers, because the quantum machines could run through every possible combination at once. A quantum computer with 300 qubits could run more calculations in an instant than there are atoms in the universe.

However, keeping qubits in superposition is challenging, and the problem grows more difficult as more qubits are involved. As such, building quantum computers that are more powerful than classical computers has proven very difficult.

Now, though, two independent teams of scientists have built a novel kind of device known as a boson-sampling computer. Described as a bridge between classical and quantum computers, these machines also make use of the bizarre nature of quantum physics. Although boson-sampling computers theoretically offer less power than quantum computers are capable of producing, the machines should still, in principle, out-perform classical computers in certain problems.

In addition, a boson-sampling computer does not require qubits. As such, "it's technologically far simpler to create than building a full-scale quantum computer," said researcher Matthew Broome, a quantum physicist at the University of Queensland in Australia.

Boson-sampling computers are actually a specialized kind of quantum computer (which is known more formally as a universal quantum computer).

"The main difference between boson-sampling computers and universal quantum computers is that boson-sampling quantum computers can't solve a universal set of problems like universal quantum computers can," Broome said. "But they are still conjectured to be able to solve problems that would be massively intractable for classical computers. Boson sampling computers are an intermediate model of a quantum computer."

Boson-sampling computers are not based on qubits, but on particles called?bosons. "In our case, we use photons," said researcher Ian Walmsley, a quantum physicist at the University of Oxford in England. Photons are the packets of energy that make up light, and are one type of boson.

Broome and Walmsley were in separate groups that each devised a boson-sampling computer, based off concepts first described by theoretical computer scientist Scott Aaronson at MIT. The computers involve multiple devices that can each generate single photons. The photons are inserted into a network where they can interact with one another. They emerge from outputs equipped with sensors to analyze the particles.

The task of calculating which outputs these photons will emerge from, an operation known as boson sampling, grows well beyond the capabilities of classical computers the more photons are involved. The new computers accurately resolved what paths the photons would take ? three photons with Broome and his colleagues' machine and four in Walmsley and his collaborators' device.

Since boson-sampling computing is in its infancy, it remains uncertain whether these computers can solve problems beyond boson sampling. Still, this research suggests that computers based on quantum physics could indeed tackle problems beyond the reach of classical computers.

Previously, there was nothing to say "that anything you can do on a quantum computer you can't do on a normal computer, which leaves in question the necessity for quantum computers," Broome said. "Now, with boson sampling, we're coming up with machines based on quantum physics that can attack problems strongly believed to be intractable for classical computers."

In the future, "it would be great to push these computers toward more photons to tackle problems that would be challenging to simulate on normal computers," study coauthor Walmsley added. Using about 20 to 30 photons would?be a problem?beyond the capabilities of classical computers.

Both research teams detailed their findings online Dec. 20 in the journal Science.

Cellular fuel gauge may hold the key to restricting cancer growth

Dec. 27, 2012 ? Researchers at McGill University have discovered that a key regulator of energy metabolism in cancer cells known as the AMP-activated protein kinase (AMPK) may play a crucial role in restricting cancer cell growth. AMPK acts as a "fuel gauge" in cells; AMPK is turned on when it senses changes in energy levels, and helps to change metabolism when energy levels are low, such as during exercise or when fasting. The researchers found that AMPK also regulates cancer cell metabolism and can restrict cancer cell growth.

The discovery was made by Russell (Rusty) Jones, an assistant professor at McGill's Goodman Cancer Research Centre and the Department of Physiology, Faculty of Medicine. Jones along with his team is the first to show that AMPK can act as a tumour suppressor in animals. The research will be published December 27 in the journal Cell Metabolism.

"Cancer is a disease in which cells lose their normal restraints on growth and start to divide uncontrollably. But, in order for cells to grow quickly they need enough energy to complete the task," Jones explained. "AMPK acts like the fuel gauge in your car -- it lets the body know when energy levels are low, and stops cell growth until there is enough gas in the tank. We wanted to see if this fuel gauge could affect the development and progression of cancer. We found that mice lacking AMPK developed tumours faster, suggesting that AMPK is important for keeping tumour development in check, at least for some types of cancer." For this study, Jones' team focused specifically on a type of blood cancer known as lymphoma. They discovered that the protein Myc, which is activated in more than half of all cancers, could promote lymphoma more rapidly when mice were deficient for AMPK.

One of the ways cancer cells support their enhanced rate of growth is by changing their metabolism, or how they generate energy. Cancer cells are different from normal cells in our body because they preferentially use sugar to fuel their growth. Jones discovered that AMPK plays a specific role in restricting cancer cells' ability to use sugar to fuel their growth. "For cancer cells with low AMPK levels, their metabolism goes into overdrive," explained Prof. Jones. "They use sugar more efficiently, allowing them to grow faster. These results suggest that turning on AMPK in cancer cells may be one way that we can restrict cancer growth."

Jones' breakthrough builds on his previous discovery that the widely prescribed medication metformin, a common diabetes drug, can restrict tumour cell growth. The results bring promise that common therapeutics that turn on AMPK and alter cellular metabolism, such as metformin, may become novel tools for cancer therapy. Jones and his colleagues at McGill are currently exploring clinical applications based on this research.

Erythritol: The Only Safe Low-Calorie Sweetener? | Care2 Healthy ...

The natural sweetener erythritol does not appear to carry the adverse effects associated with other low and non-caloric natural and artificial sweeteners and may actually have antioxidant potential. For a while it was only available in Japan but now it?s becoming more accessible. It?s found naturally in pears and grapes, but industrially we have yeast make it for us. It doesn?t cause cavities and hasn?t been implicated in some of the disorders tied to other sweeteners such as fibromyalgia (see my video Aspartame-Induced Fibromyalgia), preterm birth (Diet Soda and Preterm Birth), headaches, hypertension, brain disorders, and platelet disorders (see A Harmless Artificial Sweetener).

What about stevia? The jury is finally in. The reason it?s been such a long time coming is that research out of Japan in the ?90s found that steviosides, the active ingredient in stevia, appeared totally harmless, but in the guts of rats intestinal bacteria transformed steviosides into something called steviol, which is toxic, causing a big spike in mutagenic DNA damage (see the graph in Is Stevia Good For You?). So the question was do we have those same rat bacteria in our guts, and it turns out we do. So we now know that when we eat stevia, mutagenic compounds are produced in our colons and absorbed into our bloodstream. The only remaining question was how much.

In the World Health Organization?s evaluation of food additives, they consider up to 4 mg/kg of body weight safe. So that?s 1.8 mg per pound. If you multiply your ideal weight in pounds by 1.8, that?s about how many milligrams of stevia compounds you should stay under on an average daily basis. As long as one consumes less than, say,? two stevia-sweetened beverages a day, stevia can be considered harmless. Erithritol may be even better than harmless, though, as you can see by clicking on the NutritionFacts.org video pick above.

IFixit loves new tech devices, especially in pieces

SAN LUIS OBISPO ? Jake Devincenzi was thrilled to get his hands on Google's new Nexus 4 smartphone. He admired its sleek black case and large touch screen ? and he couldn't wait to tear it apart.

In a small room cluttered with discarded computer parts, Devincenzi picked up a blue plastic stylus and eased the tool into a seam on the side of the phone as three co-workers watched.

Minutes later, a pop. The tear-down had begun.

"We're in," he said, and grinned.

Each time Devincenzi plucked a part from the Nexus 4, he took a high-resolution photo and posted it online. By the end of the week, more than 60,000 people had scrutinized the tear-down, curious to know what was inside.

Devincenzi, a 20-year-old Cal Poly San Luis Obispo student, works part time for IFixit, a company that has less of a business mission than a manifesto: "Repair is independence. We have the right to remove 'Do Not Remove' stickers."

With the helping hands of technicians such as Devincenzi and repair guides written by volunteer tech experts, IFixit is tapping into growing frustration with cellphones, tablets and computers that, once broken, are almost impossible to fix. The company says it wants to teach people how to repair electronic devices once again ? and will sell them the tools to do it.

The company's logo is a clenched fist thrusting a wrench in the air.

"It's not like it was 10 years ago, when you could open things up and kind of half-recognize what was in there," said Sean Campbell, a technology industry analyst with the Oregon research firm Cascade Insights. "I think most people have given up."

To IFixit staff, every high-resolution photograph of naked phone bits splayed across a table is a small act of rebellion against big technology companies like Apple, Google and Samsung. Tear-downs, they say, are a rallying cry for the common man to grab a screwdriver and take back the right to do it yourself.

"We're not necessarily actively anti-Apple or anti 'the Man,'" said Scott Dingle, 25, a customer service representative. "It's more like, we train other people to do it themselves."

What is now a staff of 35 began as two people. In 2003, Cal Poly San Luis Obispo freshmen Kyle Wiens and Luke Soules started selling laptop parts out of a dorm room. When they couldn't find manufacturer-issued repair guides, they wrote their own. The first manual they posted online got 10,000 page views in the first weekend.

They moved their business to a three-car garage, then a house, then a loft-like two-story office in downtown San Luis Obispo. Most IFixit employees are younger than 35. Many grew up taking apart toasters.

Last year, IFixit earned $5.9 million in revenue by selling parts, kits and tools. Some of their more popular items include a "spudger" to adjust small wires, a five-sided screwdriver that fits Apple's proprietary screws, and a magnetic mat to keep track of tiny pieces that come loose.

Some of its tools resemble teeth-cleaning instruments. That's because the first tools IFixit used had been discarded by a dentist.

After Christmas, the company usually sees a spike in sales of parts, Wiens said. He attributes that to people who decide to repair their old gadgets if they don't get a new one as a gift.

Soules, 28, is the business mind. He runs the in-house shipping center and handles the money. The revenue he oversees covers operating costs, including salaries, tear-downs and expensive trips to buy gadgets the moment they come out.

Wiens, 28, is the chief executive and the ideas guy. In college, he dreamed of building a robot that would pick strawberries. Today he dreams of a world where computers don't wind up in landfills, but are fixed instead.

On the Friday before Thanksgiving, workers gathered at IFixit headquarters. Bikes, homemade robots and vintage pinball machines crowded the entryway. Upstairs, an employee's dog sniffed past an 8-foot-tall potato launcher and into the office break room, where Wiens' wife had prepared dinner for more than 50 people.

Spouses and significant others settled on couches to eat. The pingpong table became a dining room table. A baby cried.

Pilgrims mark Christmas in Bethlehem

At midnight mass in the Church of Nativity in Bethlehem, the cradle of Christianity, the message was of peace, love and goodwill to all mankind. NBC's Martin Fletcher reports.

Pilgrims and locals celebrated Christmas Day on Tuesday in the ancient Bethlehem church where tradition holds Jesus was born, candles illuminating the sacred site and the joyous sound of prayer filling its overflowing halls.

Overcast skies and a cold wind didn't dampen the spirits of worshippers who came dressed in holiday finery and the traditional attire of foreign lands to mark the holy day in this biblical West Bank town. Bells pealed and long lines formed inside the fourth-century Church of the Nativity complex as Christian faithful waited eagerly to see the grotto that is Jesus' traditional birthplace.

Duncan Hardock, 24, a writer from MacLean, Va., traveled to Bethlehem from the republic of Georgia, where he had been teaching English. After passing through the separation barrier Israel built to ward off West Bank attackers, he walked to Bethlehem's Manger Square where the church stands.

"I feel we got to see both sides of Bethlehem in a really short period of time," Hardock said. "On our walk from the wall, we got to see the lonesome, closed side of Bethlehem ... But the moment we got into town, we're suddenly in the middle of the party."

Bethlehem lies 6 miles south of Jerusalem. Entry to the city is controlled by Israel, which occupied the West Bank in 1967.

Hardock's girlfriend, 22-year-old Jennifer Gemmell of Longmont, Colorado, compared the festive spirit in Manger Square on Christmas Eve, saying "it's like being at Times Square at New Year's."

On Monday evening, Pope Benedict XVI prayed that Israelis and Palestinians live in peace and freedom, and asked the faithful to pray for strife-torn Syria as well as Lebanon and Iraq.

He urged people to reflect upon what they find time for in their busy, technology-driven lives.

"The great moral question of our attitude toward the homeless, toward refugees and migrants takes on a deeper dimension: Do we really have room for God when he seeks to enter under our roof? Do we have time and space for him?" the pope said.

Later Tuesday, the world's Christmas focus will shift to Vatican City, where the pope will deliver his traditional "Urbi et Orbi" speech ? Latin for "to the city and the world" ? from the central balcony of St. Peter's Basilica to thousands of pilgrims, tourists and Romans gathered in the piazza below.

Ultrasound diagnoses appendicitis without X-rays

Dec. 24, 2012 ? Children suspected of having appendicitis are more likely to receive CT scans, which involve radiation, if they are evaluated at a general hospital, a new study by Washington University School of Medicine in St. Louis has shown.

Similar patients who went to St. Louis Children's Hospital were more often evaluated with an ultrasound scan, a safer option that uses sound waves instead of radiation to confirm or rule out the need for surgery to remove the appendix.

Use of either scanning technique can potentially reduce the occurrence of unnecessary surgeries and expedite the diagnosis of appendicitis. But recent reports have suggested that the radiation exposures in CT scans can significantly increase children's lifetime cancer risk. As a result, researchers are reassessing the role of CT scans and seeking ways to reduce their use.

The study appears online Dec. 24 in the journal Pediatrics.

"Appendicitis is a very tough diagnosis, because its symptoms overlap with viral infections and other problems," says first author Jacqueline Saito, MD, assistant professor of surgery. "We don't want to operate when the appendix is fine, but if we wait too long, an inflamed appendix can rupture or perforate, making recovery more complicated and much slower."

The appendix is a finger-shaped pouch that extends from the large intestine. Infection or blockage of the appendix causes appendicitis, which can lead to abdominal pain, vomiting and fever.

Saito and her colleagues analyzed case records of 423 children who had appendectomies, or surgery to remove the appendix, at St. Louis Children's Hospital. In 218 patients initially evaluated at Children's Hospital and 205 at general hospitals, researchers reviewed how the patients were evaluated for appendicitis and whether the surgery's results confirmed the diagnosis.

CT scans, which take X-ray images from multiple angles, have been the primary diagnostic scan for detecting appendicitis for many years. About 85 percent of children initially evaluated at a general hospital underwent preoperative CT scans, and 45 percent of children initially seen at St. Louis Children's Hospital had CT scans. Using ultrasound to detect appendicitis has recently become more frequent, especially at St. Louis Children's Hospital; over half of children initially seen at St. Louis Children's Hospital, compared to 20 percent at general hospitals, had preoperative ultrasound.

Only 7 percent were not scanned using either method, and 15 percent had both types of imaging.

While ultrasound scans are safer for diagnosing appendicitis in children, they must be performed and interpreted by personnel who have received specialized training and are familiar with pediatric diagnostics.

"Ultrasound scans are difficult to perform correctly in this context, and what specialists can do at Children's Hospital may not be realistic or even available in a general hospital, which doesn't care for children as often," Saito says.

Saito is currently studying the outcomes of patients whose scans ruled out an appendectomy, looking to see if they had any additional symptoms or eventually had to have their appendixes removed.

"Ultimately what we'd liked to do is learn how we can reduce our use of CT imaging without compromising patient care," she says. "We want to find ways to identify the patients who really need these scans and those who can be effectively evaluated using other methods."
